Funded by the National Institutes of Health, we bring together nationally recognized clinicians and scientists to study clinical and translational problems in addiction. As clinicians and scientists, we know firsthand the day-to-day challenges we face in the clinical management of alcohol and other drug problems in youth along with the shortcomings of the best available therapies. We are committed to advancing interventions that integrate pharmacological and behavioral interventions to achieve optimal effects on treatment outcomes. Through close partnership with Bradley Hospital — the nation’s first psychiatric hospital exclusively for children and adolescents — we deliver the highest quality clinical services to teens who struggle with mental health and alcohol, marijuana, or other drug-related issues and their families. This clinical extension of our research program is designated a model treatment program by the Substance Abuse and Mental Health Services Administration. As part of Brown University Center for Alcohol and Addiction Studies and the Warren Alpert Medical School, we are dedicated to the training of the next generation of clinicians and researchers focused on addiction and its complications.       <image:title>Publications - Understanding how treatments for adolescent substance use work: Current status and new directions</image:title>
      <image:caption>Samuel Meisel, Melissa Pielech, Molly Magill, Kelsey Sawyer, &amp; Robert Miranda Jr. A new systematic review by our team summarizes over 20 years of clinical trial research focused on uncovering how treatments for adolescent alcohol and other drug use produce their beneficial effects. This work, published in the leading scientific journal Clinical Psychology: Science and Practice, organized findings using the classification system put forth by the National Institutes of Health Science of Behavior Change (SOBC) program. We found the most consistent support for self-regulation and mixed support for interpersonal and social processes, and findings suggest less support for mediators of marijuana use outcomes relative to alcohol and multiple drug outcomes. The quality of prior studies was generally poor, however, which dampened our ability to draw any strong conclusions. This work highlights the need for systematic efforts to studying mediators of adolescent substance use treatment. Adoption of experimental medicine framework and a clearly articulated ontology of treatment mediators, can provide a unified methodology to understanding how adolescent substance use treatments work and inform clinical practice and guide intervention refinement.</image:caption>

      <image:caption>Hayley Treloar Padovano &amp; Robert Miranda Jr. Craving or the urge to drink alcohol is a common trigger for relapse. Preclinical research shows that, among alcohol-dependent animals, longer periods of abstinence enhance craving, especially in responses to the sight and smell of alcohol, and studies show similar effects in humans. A new study published in Addiction Biology, an international scientific journal focused on neuroscience research aimed toward advancing our understanding of aspects on the action of drugs of abuse and addictive processes, examined for the first time whether “incubation of craving” exists across ages, from adolescence through adulthood, and compared across individuals with and without alcohol dependence. Findings demonstrate that incubation of cue-elicited craving occurs in persons with alcohol dependence and applies regardless of the drinker’s age.</image:caption>

Yet our understanding of how social contexts change during treatment remains quite limited. A new study published in the Journal of Consulting and Clinical Psychology showed that young people decreased their time spent with friends and siblings who use marijuana and increased their time with non-using friends or alone during treatment. Time with parents or non-using siblings was unchanged. Spending more time with friends or siblings who use was linked with greater cannabis craving and use. This research supports increased attention to youths’ changing social contexts to enhance treatment success.</image:caption>

      <image:caption>Ethan Mereish, Robert Miranda Jr., Yang Liu, &amp; David Hawthorne Emotions play a key role in mental health and general well-being. Research shows that teens who identify as sexual minorities are especially prone to a host negative mood states compared to their heterosexual peers. A new study by our group leveraged daily diary assessment methods and revealed for the first time that stigma-related or other minority stressors confer significant risk for disruptions in affect and mood in the daily lives of sexual minority teens. Minority stressors related to teens’ other marginalized identities beyond their sexual identities, such a race or ethnicity, also triggered spikes in negative affect. Published in the Journal of Counseling Psychology, a leading publication by the American Psychological Association, this study underscores the need to tailor interventions to help adolescents to cope with negative affect specific to their sexual and other intersecting identities.</image:caption>

      <image:title>Publications - How adolescents’ working memory abilities relate to their alcohol craving in real-life contexts depends on biological sex</image:title>
      <image:caption>Hayley Treloar Padovano &amp; Robert Miranda Jr. Adolescence is characterized by heightened sensitivity to rewards, social influences, and stress that extends into the mid-twenties. In a recent study, our group leveraged mobile-assessment technology to examine whether alcohol cues, peers, and stress elicit alcohol craving among youth in daily life. Published in the international scientific journal Drug and Alcohol Dependence, this study found that on the whole, when youth were not drinking, they experienced heightened alcohol craving during stressful moments, while with their peers, and when faced with alcohol-related cues, such as the sight of an alcoholic beverage. For males but not females, greater capacity for cognitive control buffered against stress-induced alcohol craving in daily life. These findings suggest that sex-related differences in alcohol problems emerging in later adolescence could be influenced by the interplay of cognitive control and in-the-moment contextual factors.</image:caption>

He directs a federally funded research program on the treatment of young people with substance use and co-occurring mental health disorders, with a special focus on testing new pharmacological interventions. He has served as principal investigator, co-investigator, or mentor on nearly 50 federally funded research projects, and he has developed several novel assessment and intervention approaches for adolescents with substance use and co-occurring mental health disorders. In 2016 and 2020, Dr. Miranda was honored to receive the Mark Wood Outstanding Mentor Award, nominated by postdoctoral fellows. This award recognizes his drive to provide high-quality mentorship and support to early career clinical investigators. He has served on numerous study sections at the National Institutes of Health and currently chairs the Treatment, and Health Services Research Review Subcommittee (AA-3) at the National Institute on Alcohol Abuse and Alcoholism. In addition to his scholarly work, Dr. Miranda founded and directs the intensive adolescent substance abuse treatment program (Vista) at Bradley Hospital. The program is the premier adolescent and emerging adult substance abuse treatment service and training clinic in the region, and it recently received recognition as a model treatment program for youth with co-occurring disorders by the Substance Abuse and Mental Health Services Administration. As both a clinician and a scientist, he understands intimately the day-to-day clinical management of substance use disorders in youth along with the shortcomings of the best available therapies and areas that warrant more research attention. He completed his clinical residency at the Warren Alpert Medical School of Brown University and a postdoctoral fellowship at the Brown University Center for Alcohol and Addiction Studies. His research focuses on substance use and how substance use disorder manifests in the natural environment. He uses ecological momentary assessment (EMA) to study substance use and co-use amongst people who both are and are not experiencing use-related problems. Much of his work has focused on alcohol, opioid, and cannabis use and co-use, but he is also interested in the use of other substances. His work also examines the effectiveness and feasibility of mobile health (mHealth) interventions to address and prevent substance use in daily life. A second and related focus of Dr. Carpenter’s research is applying models of negative reinforcement to shed light on the processes through which aversive internal experiences (e.g., negative affect, physical pain) lead some individuals to use substances or engage in self-destructive behaviors, such as non-suicidal self-injury. He is an expert in conducting clinical trials focused on medication development for addiction. He has collaborated with Dr. Miranda on six NIH-funded clinical trials of novel medications over the past decade. In addition to medication development, his previous and current work includes an NIAAA-funded K23 award and translational research to implement screening, brief interventions, and referrals to treatment protocols in pediatric emergency departments. His other major focus is multi-center research, participating in two federally funded research networks. Since 2011, his emergency department has been a participating Pediatric Emergency Care Applied Research Network (PECARN) site. Composed of 18 academic pediatric emergency departments and 3 emergency medical services sites across the US, PECARN is a federally funded, multi-center research network that has performed seminal, high impact research since 2001. Since September 2016, he has been a Co-PI of an NIH ECHO (Environmental Influences on Child Health Outcomes) award (UG1 OD024951). He provides direct patient care through Bradley Vista, Thundermist Health Centers, and the Gender &amp; Sexuality Clinic at Hasbro Children’s Hospital. He is involved with the American Academy of Pediatrics (AAP), frequently reviewing policy and publications related to LGBTQ health.       <image:caption>Nurse Practitioner, Bradley Hospital Expertise: Assessment and treatment of adolescent substance use and related psychiatric disorders; pharmacotherapy Dr. Weitz is a board-certified advanced practice registered nurse who specializes in the assessment and treatment of adolescents and emerging adults who struggle with substance use and related mental health conditions. She has dedicated her career to advancing and delivering evidence-based behavioral health care for adolescents. For nearly two decades, Dr. Weitz has provided the highest-quality direct clinical care to youth and their families across a variety of settings in New York, Massachusetts, and Rhode Island. Dr. Weitz earned her doctorate in nursing from the University of Massachusetts in Amherst and completed her practicum training in May of 2020. Prior to her doctoral studies, Dr. Weitz was a registered nurse providing care in the child and adolescent psychiatric inpatient setting at Bellevue Hospital, as well as case management in the home-care setting for comorbid psychiatric and medical conditions. From both a professional and personal capacity, she is committed to lead with authenticity and integrity and to assess and evaluate clients and their families from a biopsychosocial lens. Importantly, Dr. Weitz is devoted to the implementation of evidence-based practice while inspiring hope and resilience. In 2018, while completing her doctoral degree, Dr. Weitz joined the Bradley Vista team at Bradley Hospital where, with Dr. Miranda, she helps lead the treatment team for the intensive outpatient program.
